Home Health Registered Nurse RN Part Time PRN

Job Summary
Conduct comprehensive in-home assessments to evaluate physical, emotional, and environmental needs, translating findings into patient-centered care plans. Deliver individualized, one-on-one nursing care including wound care, infusions, medication management, and chronic disease management tailored to patient goals. Educate patients and caregivers on symptom and medication management while monitoring progress to adjust plans and prevent hospitalizations. Coordinate care across providers to ensure continuity and complete timely, accurate documentation supporting quality outcomes. Perform Monday through Friday visits across Britt, Virginia, Eveleth, Mt. Iron, Aurora, Hoyt Lakes, and Gilbert communities. Requires active RN license with one year of clinical experience, CPR certification, and reliable transportation.
Required Qualifications
- Active Registered Nurse (RN) license in the state of practice
- At least one (1) year of clinical nursing experience
- Patient-focused mindset

- Motivated by delivering high-quality care
- Current driver's license
- Reliable transportation
- Auto insurance
- CPR certification (or ability to obtain certification)
- Compliance with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing
