Home Health Registered Nurse RN Part Time PRN

Develop a plan of care through physician orders, client input, and nursing assessment to include medical interventions and measurable goals. Provide skilled nursing care in patients' homes using skills such as phone triage, wound care, infusions, and catheter care while educating clients and training primary caregivers. Coordinate the continuum of hands-on client care through documentation and timely communication with the client's physician and other caregivers. Adhere to confidentiality standards and professional boundaries at all times. PRN visits required Monday through Friday in the Baldwin area. Active RN license and valid CPR are required; Medicare Skilled Nursing experience and a basic understanding of Oasis are preferred.
