Home Health Registered Nurse RN Part Time PRN

Develop a plan of care through physician orders, client input, and nursing assessment to include medical interventions and measurable goals and outcomes. Educate clients and their family members based on specific needs while properly orienting and training primary caregivers. Provide skilled nursing care in patients' homes using skills such as phone triage, wound care, infusions, and catheter care. Coordinate the continuum of hands-on client care through documentation and timely communication with physicians and other caregivers. Schedule includes part-time visits Monday through Friday with an on-call rotation.
