Home Health Registered Nurse RN Part Time

Job Summary
Deliver one-on-one, skilled nursing care in the home, conducting comprehensive assessments to evaluate physical, emotional, and environmental needs. Provide interventions including wound care, infusions, medication management, and chronic disease management tailored to patient goals. Educate patients and caregivers on symptom management and daily care while monitoring progress to prevent hospitalizations. Coordinate care across providers and complete timely documentation reflecting the full clinical picture. Work Monday through Friday during daytime visits with participation in a team shared on-call rotation within the Detroit Lakes territory. Requires an active RN license, one year of clinical experience, CPR certification, and reliable transportation.
Required Qualifications
- Active Registered Nurse (RN) license in the state of practice
- At least one (1) year of clinical nursing experience
- Patient-focused mindset

- Motivated by delivering high-quality care
- Current driver's license
- Reliable transportation
- Auto insurance
- CPR certification (or ability to obtain certification)
- Compliance with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing
