Job Summary
Conduct comprehensive nursing assessments, including MDS-HC evaluations, and develop individualized care plans for Adult Family Care and Group Adult Foster Care members. Supervise LPNs and caregivers while reviewing documentation to ensure compliance with MassHealth and state regulations. Coordinate care with physicians and community partners, educate members on medication and wellness, and participate in interdisciplinary team meetings and quality assurance audits. Manage chronic illness monitoring and emergency preparedness planning within assigned service areas.
Required Qualifications
- Current Massachusetts Registered Nurse (RN) license in good standing
- Minimum of one year of clinical nursing experience
- Strong assessment, critical thinking, and clinical documentation skills
- Excellent communication and organizational skills
- Ability to work independently while managing multiple priorities
- Reliable transportation
- Willingness to travel throughout assigned service areas
- Current CPR certification
Desired Qualifications
- Experience in Adult Family Care (AFC), Group Adult Foster Care (GAFC), home care, community health, case management, hospice, long-term care, or behavioral health
- Experience completing MDS-HC assessments
- Knowledge of MassHealth regulations and AFC/GAFC standards
- Experience supervising nursing staff and caregivers
- Familiarity with electronic health record (EHR) systems and EVV documentation
