Job Summary
Coordinate care plans, admit patients, and collaborate with interdisciplinary teams to promote optimal health outcomes in home settings. Provide skilled nursing procedures, medications, and treatments as prescribed by physicians, while completing initial OASIS assessments and establishing nursing diagnoses. Arrange necessary equipment and supplies, supervise LPNs and home health aides, and educate patients on self-care and preventive measures. Continuously review patient care documentation, communication, and discharge planning per CMS and Colorado guidelines. Participate in on-call duties and support the Quality Management program. Maintain knowledge of state, federal, and local regulations. Prepare clinical notes and update physicians regularly.
Required Qualifications
- Current Colorado RN License required, or compact covering Colorado
- Valid Driver's License & clean Motor Vehicle Record (MVR)
Desired Qualifications
- One year nursing experience in home health preferred
