Job Summary
Provide home-based care to patients with illnesses, injuries, and chronic diseases, often following hospital discharge. Examine patients, perform assessments, document progress, and report changes to physicians. Educate patients on their care plans, medications, therapy, diet, and exercise. Coach family members on patient plans, medication management, and home safety. Work with a team of physicians and oversee LPNs, CNAs, and aides. Ensure goals are met and coordinate patient discharge. Full-time or part-time roles (minimum 5 visits or 10 hours/week). Compensation ranges from $50–$150 per visit plus mileage and travel reimbursement.
Required Qualifications
- Active Registered Nurse (RN) License in Colorado
- Graduate of an accredited school of nursing
- At least 1200 hours of experience as an RN
- Health care CPR/BLS Certified (demonstration course required)
- Pass a background check and drug test (we are required to test for THC)
Desired Qualifications
- Home health care experience preferred
