Job Summary
Home Health Registered Nurse (RN) to provide one-on-one home care, conduct initial and ongoing comprehensive assessments, develop and implement care plans in collaboration with the Case Manager, coordinate patient discharge from services, and perform skilled interventions during home visits. Requires graduate of an accredited nursing program with an active Arizona RN license, CPR certification, valid TB screening, and a fingerprint card. Responsibilities include patient care according to the plan of care, goal clarification with patients/families, and strong communication and critical thinking to support safe, effective home health nursing. The role emphasizes individualized patient care, travel/mileage reimbursement, competitive weekly pay, and opportunities for continuing education benefits.
Required Qualifications
- Graduate of an accredited nursing program
- Active RN license in Arizona
- One (1) Home health experience preferred but not required
- Valid Arizona Fingerprint Card
- CPR Certification (demonstration course required)
- Must have a valid TB screening
- Proof of flu vaccination, during Flu season in the months of October-March (optional)
- Knowledge of state and federal healthcare laws and regulations
- Demonstrated clinical proficiency, critical thinking skills, good communicator and compassionate
