Home Health Registered Nurse (RN) - Hayward

Home Health RN monitors patients in their homes, develops individualized plans of care with the primary care physician, functions as a case manager, and delivers specialized home health services including wound therapy, post-surgical and ostomy care management. The role supports independent practice with internal operational support for tasks like medication reconciliation, DME coordination, MD signatures, coding, and lab follow-up, enabling flexible scheduling and patient visits in the homes of patients within Hayward area and surrounding CA locations. Requires active CA RN license, 1 year of RN experience in relevant settings, and willingness to train onsite; schedule is full-time with non-traditional shifts possible (e.g., Thursday-Monday) and on-site training offered.
