Job Summary
Home Health Registered Nurse delivers in-home patient care, assessing new patients and determining care needs; builds care plans with patients and families; administers medications and treatments; educates patients and families on home care management; updates care plans as patient status changes; coordinates with physicians, discharging patients when ready; collaborates with community health partners and arranges required equipment or services; conducts follow-ups with home health aides every two weeks; requires driving with a state license and CPR; retention bonus included disbursed over 18 months; reports to Branch Director.
Required Qualifications
- Graduate of an accredited school of nursing
- Current licensure in state and CPR certification
- Must be a licensed driver with insured automobile
- Excellent observation, verbal and written communication skills
- Basic math skills; nursing skills per competency checklist
