Job Summary
Assess each new patient's condition, history, and home care needs; build and update the care plan with the patient and family; administer medications and treatments as prescribed; teach patients and families how to manage care at home; coordinate with physicians and send updates at least every 60 days and with community health partners; plan and carry out discharge when the patient is ready; arrange any equipment or services the patient needs; check in on home health aide care every two weeks.
Required Qualifications
- Graduate of an accredited school of nursing
- One (1) to two (2) years of recent acute care experience in an institutional setting
- Current licensure in state and CPR certification
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
