Job Summary
Deliver one-on-one skilled nursing care in the home, conducting comprehensive assessments to evaluate physical, emotional, and environmental needs. Provide interventions including wound care, infusions, medication management, and chronic disease management tailored to patient goals. Educate patients and caregivers on symptom and medication management, while monitoring progress to prevent avoidable hospitalizations. Coordinate care across providers and complete timely documentation supporting quality outcomes. Work full-time daytime hours Monday through Friday with participation in a team shared on-call rotation in the Des Moines territory. Requires an active RN license, one year of experience, CPR certification, and a current driver's license.
Required Qualifications
