Job Summary
Provide one-on-one skilled nursing care in patients' homes as part of Aveanna’s Home Health team. Perform in-home assessments, wound care, infusions, catheter care, medication management, post-operative and chronic disease management, and educate patients and caregivers to manage symptoms and daily care. Coordinate with the interdisciplinary team, monitor patient progress, adjust plans as needed, and complete timely documentation to support quality outcomes. Requires an active RN license in the state of practice, at least 1 year of clinical nursing experience, a valid driver’s license with reliable transportation and auto insurance, and CPR certification (or the ability to obtain it).
Required Qualifications
