Job Summary
Deliver one-on-one, skilled nursing care in the home, conducting comprehensive assessments to evaluate physical, emotional, and environmental needs. Provide interventions including wound care, infusions, and medication management while educating patients and caregivers on symptom and daily care management. Monitor patient progress, communicate with physicians and interdisciplinary teams to adjust plans, and coordinate care across providers to ensure continuity and prevent avoidable hospitalizations. Complete timely, accurate documentation reflecting the full clinical picture. Full-time Monday through Friday daytime visits with participation in team shared on-call rotation. Requires an active RN license with at least one year of clinical experience, a current driver's license, and CPR certification.
Required Qualifications
- Active Registered Nurse (RN) license in the state of practice
- At least one (1) year of clinical nursing experience
- Current driver's license
- Reliable transportation
- Auto insurance
- CPR certification (or ability to obtain certification)
- Compliance with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing
