Job Summary
Deliver individualized, one-on-one nursing care in the home, conducting comprehensive assessments to evaluate physical, emotional, and environmental needs. Provide skilled interventions for wound care, infusions, medication management, and chronic disease management while educating patients and caregivers on symptom and daily care management. Monitor patient progress, communicate with physicians and interdisciplinary teams to adjust plans, and coordinate care across providers to ensure continuity and prevent hospitalizations. Complete timely, accurate documentation reflecting the full clinical picture. Requires an active RN license with at least one year of experience, a current driver's license, and CPR certification. Offers a $15,000 sign-on bonus and full-time status with competitive pay, medical/dental/vision insurance, 401(k) match, tuition reimbursement, and free continuing education.
Required Qualifications
- Active Registered Nurse (RN) license in the state of practice
