Job Summary
Deliver one-on-one skilled nursing care in the home, conducting comprehensive assessments to evaluate physical, emotional, and environmental needs. Provide interventions including wound care, infusions, and medication management while educating patients and caregivers on symptom management. Monitor patient progress closely, communicate with physicians and the interdisciplinary team to adjust plans, and coordinate care across providers to ensure continuity. Complete timely, accurate documentation that reflects the full clinical picture and supports quality outcomes. Requires an active RN license with at least one year of experience, CPR certification, and a current driver's license. Full-time position in La Crosse, Onalaska, and surrounding communities with a $15,000 sign-on bonus.
Required Qualifications
- Holds an active Registered Nurse (RN) license in the state of practice
- at least one (1) year of clinical nursing experience
- Is patient‐focused, organized, and motivated by delivering high-quality care
- Has a current driver's license
- Has reliable transportation
- Has auto insurance
- Is CPR certified
