Job Summary
Deliver one-on-one, skilled nursing care in the home, conducting comprehensive assessments to evaluate physical, emotional, and environmental needs. Provide interventions including wound care, infusions, medication management, and chronic disease management tailored to patient goals. Educate patients and caregivers on symptom and medication management while monitoring progress to prevent hospitalizations. Coordinate care across providers and complete accurate documentation supporting quality and compliance. Requires an active RN license with one year of clinical experience, a current driver's license, and CPR certification. Full-time position in La Crosse, Onalaska, and surrounding communities with a $15,000 sign-on bonus.
Required Qualifications
- Active Registered Nurse (RN) license in the state of practice
- At least one (1) year of clinical nursing experience
