Job Summary
Deliver one-on-one, skilled nursing care to patients and families in the McCalla & Bibb County territory, conducting comprehensive in-home assessments and translating findings into patient-centered care plans. Provide interventions including wound care, infusions, medication management, and chronic disease management while educating caregivers on symptom and daily care management. Monitor patient progress, communicate with physicians and the interdisciplinary team to adjust plans, and coordinate care across providers to ensure continuity and prevent avoidable hospitalizations. Complete timely, accurate documentation reflecting the full clinical picture. Monday-Friday schedule with on-call rotation; $10,000 sign-on bonus available.
Required Qualifications
- Active Registered Nurse (RN) license in the state of practice
- At least one (1) year of clinical nursing experience
- Patient-focused mindset

- Motivated by delivering high-quality care
- Current driver's license
- Reliable transportation
- Auto insurance
- CPR certification (or ability to obtain certification)
- Compliance with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing
