Job Summary
Conduct comprehensive in-home assessments to evaluate physical, emotional, and environmental needs, translating findings into patient-centered care plans. Deliver individualized, one-on-one nursing care including wound care, infusions, medication management, and chronic disease management tailored to patient goals. Educate and empower patients and caregivers through clear instruction and support to help them manage daily care at home. Monitor patient progress closely, identify changes in condition early, and communicate effectively with physicians and the interdisciplinary team to adjust plans and prevent hospitalizations. Coordinate care across providers to ensure continuity and complete timely, accurate documentation supporting quality and compliance. Work Monday through Friday with an on-call rotation in McCalla and Bibb County.
Required Qualifications
- Active Registered Nurse (RN) license in the state of practice
- At least one (1) year of clinical nursing experience
- Current driver's license
- Reliable transportation
- Auto insurance
- CPR certification (or ability to obtain certification)
- Compliance with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing
