Job Summary
Develop a plan of care through physician orders, client input, and nursing assessment to include medical interventions and measurable goals. Educate clients and family members based on specific needs while properly orienting and training primary caregivers. Provide skilled nursing care in patients' homes using skills such as phone triage, wound care, infusions, and catheter management. Coordinate the continuum of hands-on client care through documentation and timely communication with the client's physician and other caregivers. Monday through Friday full-time visits with a shared on-call rotation in Murray County.
Required Qualifications
- An active RN License in the state of application
- Valid CPR
- Drivers License
- Proof of automobile insurance
- Must be able to lift 50 lbs
- Must be available for weekend shifts
- Must be able to effectively lead and coach others

Desired Qualifications
- Medicare Skilled Nursing experience
- Basic understanding of Oasis
- 1-year RN experience in a health care setting
