Job Summary
Deliver individualized, one-on-one nursing care in the home, conducting comprehensive assessments to evaluate physical, emotional, and environmental needs. Provide skilled interventions including wound care, infusions, medication management, and chronic disease management tailored to patient goals. Educate and empower patients and caregivers through clear instruction, while monitoring progress and communicating effectively with physicians to adjust plans and prevent avoidable hospitalizations. Coordinate care across providers to ensure continuity and complete timely, accurate documentation supporting quality and compliance. Work full-time Monday through Friday with a shared on-call rotation in Citronelle, Wilmer, and North Mobile County. Requires an active RN license with one year of clinical experience, a current driver's license, and CPR certification.
Required Qualifications
- Active Registered Nurse (RN) license in the state of practice
- At least one (1) year of clinical nursing experience
