Job Summary
Provide one-on-one skilled nursing care in patients' homes, including comprehensive in-home assessments, individualized care plans, wound care, infusions, catheter care, medication management, post-operative and chronic disease management, and caregiver education to promote recovery, safety, and independence. Coordinate with physicians and the interdisciplinary team, monitor progress, document thoroughly to support quality and compliance, and participate in on-call rotation. Regular daytime home visits and an on-call schedule in the Detroit Lakes, MN area. Requires active RN license and at least 1 year of clinical nursing experience, valid driver’s license and reliable transportation, CPR certification (or ability to obtain).
Required Qualifications
- Holds an active Registered Nurse (RN) license in the state of practice
- at least one (1) year of clinical nursing experience
- Is patient-focused, organized, and motivated by delivering high-quality care
- Has a current driver’s license, reliable transportation, and auto insurance
- Is CPR certified (or able to obtain certification)
