Job Summary
Conduct initial patient assessments to generate personalized care plans in collaboration with physicians and the interdisciplinary team. Provide community-based care to help patients regain independence, while observing signs and symptoms and reporting changes to health professionals. Teach, supervise, and counsel patients and families regarding home nursing procedures and care needs as appropriate. Exhibit critical thinking to drive continuous improvement of services and the agency. Perform home visits and attend office meetings as indicated, adhering to a full-time schedule of 30 points per week.
Required Qualifications
- Active Michigan Registered Nurse (RN) license
- Reliable transportation
- Valid driver's license and auto insurance in your name as a driver
