Home Health Registered Nurse RN - Full Time - 10K Bonus

Conduct initial patient assessments to generate personalized care plans in collaboration with physicians and the interdisciplinary team. Provide community-based care to help patients regain independence while observing signs and symptoms to report changes to health professionals. Teach, supervise, and counsel patients and families regarding home nursing procedures and care needs as appropriate. Exhibit critical thinking to drive continuous improvement of services and the agency. Participate in necessary in-service programs and attend office meetings as indicated. Requires an active Michigan RN license, reliable transportation, and valid driver's license.
