Job Summary
Deliver one-on-one, skilled nursing care in the home, conducting comprehensive assessments to evaluate physical, emotional, and environmental needs. Provide interventions including wound care, infusions, medication management, and chronic disease management tailored to patient goals. Educate patients and caregivers on symptom management and daily care while monitoring progress to prevent hospitalizations. Coordinate care across providers and complete timely documentation to support quality and compliance. Requires an active RN license with one year of experience, CPR certification, and a valid driver's license. Required Qualifications
- Holds an active Registered Nurse (RN) license in the state of practice
- At least one (1) year of clinical nursing experience
- Is patient-focused, organized, and motivated by delivering high-quality care
- Has a current driver's license, reliable transportation, and auto insurance
- Is CPR certified (or able to obtain certification)
- Comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing
