Job Summary
Deliver one-on-one, skilled nursing care in the home that promotes patient recovery, safety, and independence. Conduct comprehensive in-home assessments to evaluate physical, emotional, and environmental needs, then translate findings into thoughtful care plans. Provide interventions including wound care, infusions, medication management, and chronic disease management tailored to each patient's goals. Educate patients and caregivers on symptom and medication management while monitoring progress to prevent avoidable hospitalizations. Coordinate care across providers and complete timely, accurate documentation supporting quality and compliance. Requires an active RN license with one year of clinical experience, CPR certification, and reliable transportation. Required Qualifications
- Holds an active Registered Nurse (RN) license in the state of practice
- at least one (1) year of clinical nursing experience
- Is patient-focused, organized, and motivated by delivering high-quality care
- Has a current driver's license, reliable transportation, and auto insurance
- Is CPR certified (or able to obtain certification)
