Home Health Registered Nurse RN Full Time 10K Bonus

Provide in-home skilled nursing care as the primary clinical partner for patients and families, delivering one-on-one nursing services in the home that promote recovery, safety, and independence. Conduct in-home assessments to evaluate physical, emotional, and environmental needs and translate findings into patient-centered care plans. Perform skilled nursing interventions including wound care, infusions, catheter care, medication management, post-operative care, and chronic disease management. Educate and empower patients and caregivers through clear instruction and support to manage symptoms, medications, and daily care at home. Monitor patient progress, identify changes in condition, and communicate with physicians and the interdisciplinary care team to adjust plans and prevent avoidable hospitalizations. Coordinate care across providers to ensure continuity and high-quality outcomes, and complete timely, accurate documentation to support quality, compliance, and measurable outcomes. Must hold an active RN license in the practice state, have at least one year of clinical nursing experience, possess a valid driver’s license and auto insurance, be CPR certified (or able to obtain), and be eligible to work in the United States.
