Job Summary
Conduct home health care services to meet clinical needs of individuals in facilities and homes. Regularly re-evaluate patient nursing needs, initiate care plans, and incorporate therapeutic, preventive, and rehabilitative actions while involving patients and families. Administer medications and treatments as prescribed, counsel patients and families, and provide health care instructions. Identify and implement discharge planning needs. Prepare clinical notes, communicate with physicians regarding patient status and orders, and coordinate care with community health-related persons. Participate in on-call duties and ensure availability of equipment and services. Requires a current RN IL license, 2+ years home health experience, and ability to lift and transfer patients. Full-time role with benefits including health insurance, dental, and mileage reimbursement.
Required Qualifications
- Minimum of 2 years of home health care experience
- Current RN IL license
- CPR certification
- Excellent observation skills
- Excellent verbal and written communication skills
- Problem solving skills
- Basic math skills
- Nursing skills per competency checklist
- Must be a licensed driver
- Must have an automobile that is insured in accordance with state/or organization requirements
- Must have an automobile that is in good working order
- Able to lift, position or transfer patients
- Able to lift supplies and equipment
- Considerable reaching, stooping, bending, kneeling or crouching
- Visual acuity and hearing to perform required nursing skills
- Must be available for on-call duties as defined by the on-call policy
