Job Summary
Conduct home health care services to meet the clinical needs of individuals within facilities and their homes. Regularly re-evaluate patient nursing needs, initiate care plans, and administer medications as prescribed. Counsel patients and families, provide health care instructions, and coordinate discharge planning. Prepare clinical notes, communicate with physicians regarding patient status changes, and ensure necessary equipment and services are available. Participate in on-call duties and work a full-time schedule with mileage reimbursement. Requires a current RN IL license, 2 years of home health experience, and a licensed driver's license.
Required Qualifications
- Minimum of 2 years of home health care experience
- Current RN IL license
- CPR certification
- Excellent observation skills
- Excellent verbal and written communication skills
- Problem solving skills
- Basic math skills
- Nursing skills per competency checklist
- Licensed driver
- Automobile insured in accordance with state/or organization requirements
- Automobile in good working order
- Ability to walk

- Visual acuity to perform required nursing skills
