Home Health Registered Nurse (RN) — Field Visits | Lake County, IL

Job Summary
Provide skilled nursing visits to patients in their homes across Lake County, IL, exercising autonomy in field nursing with self-scheduling and a supportive agency. Lead nursing care from admission through discharge, develop and update the plan of care based on clinical findings, and serve as the patient advocate while coordinating with physicians, referral sources, caregivers, and facility staff. Oversee the nursing clinical care team (LPNs and HHAs), coordinate care across the multidisciplinary team, and complete timely, accurate skilled nursing documentation in the EMR (OASIS documentation). Maintain compliance and quality through ongoing assessment, planning, implementation, and evaluation of patient care.
Required Qualifications
- Current, active Illinois RN license in good standing
- Current CPR/BLS certification
- Valid driver's license, reliable vehicle, and auto insurance (required for travel between patient homes)
- Prior nursing experience; home health experience and OASIS familiarity preferred
- Comfort with clinical documentation software; WellSky Home Health EMR experience is a plus, but not required
- Strong assessment, problem-solving, and communication skills
- Ability to perform the essential functions of field nursing, with or without reasonable accommodation
