Home Health Registered Nurse RN Contingent

Conduct initial patient assessments to generate personalized care plans in collaboration with physicians and the interdisciplinary team. Provide community-based care to help patients regain independence, while observing signs and symptoms to report changes to health professionals. Teach, supervise, and counsel patients and families regarding home nursing procedures and care needs as appropriate. Exhibit critical thinking to continuously improve services and agency standards. Participate in necessary in-service programs and attend office meetings as indicated. Maintain reliable transportation and agency-required auto insurance for all home visits and travel.
