Job Summary
Home Health RN (Weekend, Field) for Conejo Valley area. Provides patient-centered home health care, develops individualized care plans with physicians, functions as case manager, and delivers services including wound therapy, post-surgical care, and ostomy care management. Requires independent scheduling, ability to train onsite, and a CA RN license with CPR/BLS. Per diem schedule with options for non-traditional days; travel within service area to patients’ homes. Equipment and administrative support provided to enable autonomous patient visits.
Required Qualifications
- Minimum 1 year RN experience (preferred in acute, SNF, Home Health or Hospice)
- Active CA Registered Nursing License
- AA, BSN, MSN
- Reliable Transportation
- Current CPR or BLS Card
- Must train onsite for 5 days Mon-Fri 9am-5:30pm in San Ramon
