Job Summary
Assess patients' physical and psychosocial needs, develop care plans with families and physicians, and manage assigned caseloads within a team model. Provide professional nursing care, complete physician orders, OASIS documentation, and medication reconciliation while maintaining safety protocols. Collaborate with interdisciplinary teams, resolve complex problems, and triage visits based on urgency. Provide rotating coverage for evenings, nights, weekends, and holidays.
Required Qualifications
- Massachusetts RN Licensure
- NH Licensure
- Valid state issued Driver's License
- One (1) year of acute medical/surgical nursing experience
- Basic Life Support Certification
- Bachelor of Science in Nursing (BSN)
- Specialty nursing certifications
- Three (3) years of acute nursing including medical/surgical, oncology, hospice, or home health
- Experience with IV and complex wound care
