Home Health Registered Nurse (RN) - Bilingual (Spanish/English)

Conduct home visits to assess patient conditions, provide skilled interventions, and teach as ordered. Complete initial and ongoing comprehensive assessments, develop care plans in collaboration with Case Managers, and coordinate patient discharge. Participate with clients in clarifying goals and incorporate these into service plans. Maintain fluency in English and Spanish to effectively communicate with Spanish-speaking patients and families. This role offers flexible scheduling and weekly pay with travel time and mileage reimbursement.
