Job Summary
Conduct home visits to assess patient conditions, provide skilled interventions, and teach families as ordered. Complete initial and ongoing comprehensive assessments, develop care plans in collaboration with Case Managers, and coordinate patient discharge. Participate with clients to clarify goals and incorporate them into service plans. Communicate fluently in English and Spanish to support patient care needs. Maintain active RN licensure in Arizona, hold valid fingerprint clearance, and demonstrate CPR certification.
Required Qualifications
- Bilingual (Spanish/English)
- Fluency in both English and Spanish
- Graduate of an accredited nursing program
- Active RN license in Arizona
- Valid Arizona Fingerprint Card
- CPR Certification
- Valid TB screening
- Proof of flu vaccination
- Knowledge of state and federal healthcare laws and regulations
- Demonstrated clinical proficiency
- Critical thinking skills
- Good communicator
- Compassionate
