Home Health - Registered Nurse (RN) - Anaheim

Conduct home health visits to administer IVs, change dressings, clean wounds, and perform comprehensive patient assessments. Document all findings and update physician orders through the Synergy Software portal. Collaborate with the Clinical Team Manager to initiate, re-evaluate, and revise patient care plans while coordinating with physicians and other team members for PT, OT, or ST services. Supervise Home Health Aides per regulatory requirements and manage patient transfers, resumption of care, and discharges. Maintain knowledge of state regulations and submit all related documentation. Requires current CA RN license, BLS certification, and IV/Phlebotomy credentials. Pay ranges from $50 to $140 per visit for W-2 per diem work.
