Job Summary
Home Health Registered Nurse (RN) role in Metro Detroit focused on Start of Care assessments, supervisory visits, care coordination, and patient education in the home setting. Responsibilities include SOC assessments and admissions, supervisory visits for quality and regulatory compliance, development and coordination of patient care plans, skilled nursing visits as needed, wound care including wound vac management, and tracheostomy care. Collaborates with physicians, therapists, caregivers, and the multidisciplinary team; maintains EMR documentation; adheres to agency policies and nursing standards; and supports patient-family education and ongoing care management.
Required Qualifications
- Active Registered Nurse (RN) license in the State of Michigan
- Minimum of one year of nursing experience
- Valid driver's license and reliable transportation
- Ability to work independently and manage a flexible schedule
- Strong assessment, communication, and organizational skills
- Proficiency with electronic documentation systems
- Ability to pass a criminal background check
- Authorization to work in the United States
