Job Summary
Perform comprehensive patient assessments, develop individualized care plans, and provide skilled nursing services including medication management, injections, and wound care in home settings. Monitor patient progress, communicate changes to physicians, educate caregivers on disease management, and coordinate care with interdisciplinary team members. Complete accurate EMR documentation and ensure compliance with Medicare, Medicaid, and state regulations while participating in case conferences. This role offers full-time opportunities in the Seattle and Bellevue areas with competitive pay ranging from $55 to $68 per hour.
Required Qualifications
- Active Washington State RN License
- Current BLS Certification
- Valid driver's license and insured vehicle
