Job Summary
Administer nursing care for clients in their residence, coordinate care with the interdisciplinary team, clients, and families, and complete initial and ongoing assessments including OASIS. Reevaluate client needs, develop and revise the Plan of Care, initiate preventative procedures, and plan for discharge while ensuring compliance with third-party payers. Provide effective communication to health care professionals, participate in Case Conferences, and supervise, teach, and provide clinical direction to LVN/LPNs, HHAs, and CNAs. Perform on-call responsibilities and provide on-call services as assigned. Requires current RN license, CPR, and a valid driver's license.
Required Qualifications
- One (1) year experience as an RN in a clinical care setting
- Completion of an accredited Registered Nursing Program
- Current driver's license in good standing
- Reliable transportation and auto liability insurance
- Current State professional license as an RN in any/all applicable states
- Current CPR
- Must read, write and comprehend English
- The ability to lift/transfer up to 50 lbs
- Ability to meet deadlines and patient needs, make quick decisions and allocate resources under stressful conditions
- Ability to travel locally and work a flexible schedule
