Job Summary
Provide home health RN care to geriatric patients in Bastrop/Cedar Creek, TX; perform comprehensive initial and ongoing assessments; determine hospice eligibility; apply infection control concepts; provide clinical directions to Hospice Aide and LPN/LVN; prepare progress notes; inform physicians of changes; monitor cases for third-party payor compliance; promote agency philosophy; educate patient and family about disease process, self-care, end-of-life care; determine scope and frequency of services based on acuity; initiate preventive and rehabilitative nursing procedures; training of other staff; per-visit pay of $50; bonus emphasis on independent practice and coordination within a home health/hospice team.
Required Qualifications
- Current RN license and good standing with the state nursing board
- Knowledge or experience utilizing Homecare Homebase EMR would be a plus
- Experience in home health or related healthcare setting preferred
- Strong clinical and assessment skills
- Excellent communication and interpersonal abilities
- Ability to work independently and manage time effectively
