Job Summary
Home Health Registered Nurse PRN for Pay Per Visit in Bastrop/Cedar Creek, TX area. Provide comprehensive assessments of geriatric patients, determine admission and re-certification criteria for hospice care, develop and revise the plan of care, educate patients and families on disease process, self-care, end-of-life care, nutrition, and care coordination with the IDG. Deliver nursing procedures, infection control, and standard precautions; supervise Hospice Aide and LPN/LVN to ensure continuity of service; document progress notes and communicate changes to physicians and team members; monitor outcomes and ensure compliance with third-party payors. Requires current RN license and experience in home health; pay rate is $50 per visit; placement reflects travel to Bastrop/Cedar Creek, TX and surrounding areas.
Required Qualifications
- Current RN license and good standing with the state nursing board
- Knowledge or experience utilizing Homecare Homebase EMR would be a plus
- Experience in home health or related healthcare setting preferred
- Strong clinical and assessment skills
- Excellent communication and interpersonal abilities
- Ability to work independently and manage time effectively
