Job Summary
Provide skilled nursing care to patients in their homes, including administering medications, wound care, and conducting patient assessments. Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals; monitor patient conditions and report changes; educate patients and families on disease management and treatment options; oversee Home Health Aides and ensure proper documentation. Maintain accurate records and coordinate with healthcare professionals; participate in TB and driver-safety programs as required. Home-based, one-on-one care with a focus on returning patients to independent living.
Required Qualifications
- Med surg, ICU, ER, or acute experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Valid driver's license, auto insurance and reliable transportation
