Job Summary
Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting patient assessments. Develop and implement individualized care plans with physicians, monitor patient conditions, educate patients and families on disease management and treatment options, maintain accurate records, coordinate with healthcare professionals, and oversee Home Health Aides to ensure proper documentation. This role reports to the Clinical Manager and is part of CenterWell Home Health within Humana, emphasizing one-on-one, in-home care delivered to support recovery and independence.
Required Qualifications
- Med surg, ICU, ER, or acute experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Valid driver's license, auto insurance and reliable transportation
- One year nursing experience
- Home Health experience
