Home Health Registered Nurse-PRN

Provide skilled nursing care in patients' homes, including administering medications, performing wound care, and conducting patient assessments. Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals; monitor patients' conditions and report changes. Educate patients and families on disease management, medications, and treatment options; maintain accurate records of patient care and coordinate with other healthcare professionals. Oversee the work of Home Health Aides and ensure proper documentation. This role is patient-facing; driving to patient homes is required as part of a CenterWell Home Health team. Beneficial focus areas include med-surg, ICU, ER, and acute care settings; CPR-certified RN with valid driver’s license and transportation. Pay range reflects base compensation per year for a part-time schedule.
