Home Health Registered Nurse-PRN

Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting patient assessments. Develop and implement individualized care plans in collaboration with physicians and other health professionals; monitor patient conditions and report changes. Educate patients and families on disease management, medications, and treatment options; maintain accurate records and coordinate with other healthcare professionals. Oversee Home Health Aides and ensure documentation is completed properly; report patient progress to the physician and Clinical Manager. This role is patient-facing, includes driving to patient homes, and requires collaboration within a care team.
