Job Summary
Provide skilled nursing care to patients in their homes, including administering medications, wound care, and patient assessments. Develop and implement individualized care plans with physicians and other healthcare professionals; monitor patient conditions and report changes; educate patients and families on disease management and treatment options; maintain accurate care records and coordinate with other professionals; oversee Home Health Aides and ensure proper documentation; drive to patient homes to deliver care.
Required Qualifications
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Med surg, ICU, ER, or acute experience
- Valid driver's license, auto insurance and reliable transportation

- One year nursing experience
