Job Summary
Support patient recovery and ability to stay at home by educating families on disease management and providing clinical oversight to the interdisciplinary team. Case manage patients with advanced illnesses in partnership with health systems and long-term care partners. Requires an RN license in the state of work, two years of nursing experience, and preferred home health experience. Current driver's license and CPR/BCLS certification are mandatory.
