Job Summary
Provide skilled nursing care to patients onsite in their homes, including administering medications, performing wound care, and conducting patient assessments. Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals. Monitor patients' conditions, report changes, and educate families on disease management and treatment options. Maintain accurate records and oversee Home Health Aides' documentation completion. Report patient progress to physicians and the Clinical Manager. Requires current unrestricted RN licensure, CPR certification, and a valid driver's license with auto insurance. Pay ranges from $49.00 to $69.00 per visit.
Required Qualifications
- Med surg, ICU, ER, or acute experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
