Job Summary
Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting patient assessments. Develop and implement individualized care plans while monitoring conditions and reporting changes to physicians and the Clinical Manager. Educate patients and families on disease management and treatment options, then maintain accurate records and oversee Home Health Aides. This per diem role supports patients regaining independence in the comfort of their homes as part of CenterWell's comprehensive home care team.
Required Qualifications
- Med surg, ICU, ER, or acute experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Valid driver's license
- Auto insurance
- Reliable transportation
- TB screening
