Home Health Registered Nurse-Per Diem

Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting patient assessments. Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals. Monitor patients' conditions, report changes, and educate families on disease management and treatment options. Maintain accurate records, coordinate with professionals, and report progress to the physician and Clinical Manager. Oversee Home Health Aides and monitor their documentation completion. Requires current and unrestricted RN licensure, CPR certification, and a valid driver's license with insurance. Per diem pay rate of $49.00 to $69.00 per visit.
